This waiver must be agreed to before the Participant is allowed to access any facilities or equipment. If member is under the age of 18 years, parent/guardian consents on child behalf and the below "I" refers to particiapting member

1. Fitness to Participate: I declare that I am medically and physically fit and able to participate in the Diving Tasmania sessions, including being competent to swim in deep water unassisted. 
2. Safety: I acknowledge that diving activities can be dangerous and injuries may occur.  I accept and assume the inherent risks in participating in the Diving Activities.
3. Medical Treatment: I consent to receiving any medical treatment that Diving Australia, Diving Tasmania or the Venue reasonably considers necessary or desirable for me during my participation in Diving Activities. I also agree to reimburse the relevant Organisation for any costs or expenses incurred in providing me with medical treatment.

I have read and agree to the terms and conditions of the enrolment and understand that diving can be a dangerous activity.
